Madeline has a few ongoing medical problems, because of some eggs the spider demon Anastasia Thiema laid inside of her before disappearing. Those medical problems been going on for a few months, and it seems sort of like pregnancy sometimes, but her tests all came back negative for that.
She's going to see a doctor or medical person about it, probably after her boyfriend Shir talks her into it. That's when she, and maybe her doctor, will figure out what's wrong. She's scared of telling anyone about the nature of her relationship with Anastasia, who had groomed her into that relationship, so she'll be pretty secretive about it. But she's also going to be scared about the health risks of having demon spider eggs in her.
At some point, under some kind of circumstances, she'll need therapy. I'm not sure how on board with the idea she'll be at the time (whether she goes because she wants to, or someone talks her into it and wears her down, or she's somehow required to go, I haven't decided).
It would also make sense if she told someone other than a therapist / her boyfriend

- Some medical person or people who might be involved in finding out about or removing the spider eggs
- Some kind of therapist type person who could talk to her about Anastasia
- Someone else she might share her secret with before she feels comfortable telling her boyfriend.
- Someone to go to the hospital with her when/if she gets those things taken out. This could overlap with 3.
