Reee this wasn't clear at all when I reread. This shouldn't be a change, just a clarification of how it works:

Valli's blessing is lightened in cases where he knows he's going to get hurt. If he knows someone is lifting a piercing gun to give him a piercing, his power won't interfere with it. There's a similar effect when it comes to things like surgery: As long as Valli knows it's going to happen, it's alright. In cases where he would be operated on without his consent, the doctors would have similar issues to someone trying to harm him.

Valli's blessing of protection is something that he can subconciously turn off to a limited extent. If he would ever need to be harmed, such as in the case of surgery, he could mentally brace himself and his body would allow it to happen. However, if Valli were unconscious, any doctor operating on him would have a similar issue as someone trying to harm him would.
