Hella is using money she has saved up along with setting up a payment plan to get a lower back tattoo that is imbued with magic to permanently put her in a feminine form, rather than the masculine one she currently has. It will read as follows:

Hella has a tattoo on her lower back that caused her change in appearance. This is a transformation effect, not an illusion or glamour. Her natural scent is largely unchanged and the charm has much of the same effect on her as hormone replacement therapy and reconstruction of her skeletal structure.
