As long as he is choosing to wear it, when people look at his hands or face while he signs, they will be able to understand what words he's saying as if they were fluent. However, they do not hear a voice in their heads, they merely understand. The way the charm works, people will only understand the meaning behind the words if it is a language they know, just like if he were speaking verbally. For instance, if he were to sign in DGS (German Sign Language), the person would know he's speaking German, but only understand the meaning if they know German themselves.
